To save time when coming to our office for your first appointment - 

Physical Therapy Patients will be receiving an email invitation where you will need to take the time to fill out in full your new patient forms and medical history. Accurate and complete information is critical to your care, and best assists your health care provider in coming up with the most effective treatment plan for your condition.  Please bring a form of ID, insurance card (if applicable), diagnostic reports, x-rays and prescription information from your physician, if possible.  

 

If you have not completed your forms in advance, please arrive 30 minutes prior to your scheduled appointment time as to prevent shortened treatment time. Otherwise, arrive at least 5 minutes before your appointment start time.

Personal Training Clients and Group Class Attendees may fill out the online.  Does not work when on mobile, desktop if preferred.  There is a printable option also provided.  

To Cancel or reschudule an Appointment

The ExerScience Center operates on a scheduled hourly appointment basis for private training sessions.  Therefore, when canceling or rescheduling an appointment, the client is required to provide MORE than 24-hour notice, in order to receive credit for the session.

However, if the client cancels or reschedules a session LESS than 24 hours to the scheduled time, the client shall be charged in full for that session. For example, for pay as you go personal training, the purchased amount will be nonrefundable.  For package purchases, a loss of session will incur.  

  

Please note that consistent client short notice cancellations and rescheduling does the client and Physical Therapist or Personal Trainer a disservice and will end the partnership at the end of the client's package. 

 

Exceptions will only be made in the case of a medical emergency accompanied by a doctor’s note. There is a no-refund policy on all packages purchased – a client may only receive a refund if accompanied by a doctor’s note.


Should the Physical Therapist or Personal Trainer cancel a session with less than 24-hour prior notice, the client shall be entitled to one free session upon rescheduling. The free session must be redeemed within 7 days of canceling the previous one or shall be forfeited.
 

However, should my Physical Therapist or Personal Trainer cancel a session with MORE than 24-hour notice, the training session will be rescheduled to ensure consistency and progress in my program.

Billing and insurance

Payments must be made in FULL before starting any training or classes. 

The ExerScience Center will soon be accepting Medicare.  Please stay tuned. 

 

We are an out-of-network provider at the moment.  

We recommend patients contact their insurance company to find out what their out-of-network benefits include, so they know what to expect regarding reimbursement.

 

Here are some questions for them to ask:

  • Do I have out-of-network benefits?

  • Do my out-of-network benefits include physical therapy?

  • What is my out-of-network deductible?

  • Once my deductible is met, what percentage is reimbursed?

  • What is my out-of-pocket amount for my out-of-network benefits?

  • Are there any forms I need to complete online and fax in with my invoice?

  • Clarify the mailing address that the claim needs to be sent to.

  • Any other requirements needed for reimbursement?

Patients may need to complete forms from their insurance company and mail them in with the therapy session invoice and insurance cover letter.

The patient is responsible for payment at the time of service. See payment options.   The patient will be responsible for paying at the time of service and can submit for reimbursement to their insurance company.

Personal Training and Group Class waivers can also be filled out online.  A copy will be emailed to you once completed.
